In order for Bright Start to start an enrollment or place your child/children on our waitlist, please read and complete the following documentation:

After reading and completing the previous documents, please fill out the following form and submit your completed documents at the end:

Waitlist Application

Once a spot becomes available or a start date has been discussed, staff will email the following documents. These documents are due on the first day of enrollment or prior to the date:

General Health Appraisal

CCCAP Agreement / Payment Plan(if applicable)

Immunization Record signed by doctor/primary care physician
